What Can a Knee MRI Scan Show?

MRI uses a strong magnetic field and radio waves to create images without ionising radiation.

A knee MRI scan can provide detailed views of:

  • The anterior and posterior cruciate ligaments, known as the ACL and PCL
  • The medial and lateral collateral ligaments, known as the MCL and LCL
  • The medial and lateral menisci
  • Articular cartilage
  • The quadriceps and patellar tendons

  • Muscles and surrounding soft tissues
  • The joint lining and fluid inside the knee

When Might a Knee Scan Help After a Sports Injury?

Sports-related knee problems may follow a sudden injury or develop gradually through repeated loading.

A sudden twist, direct impact, rapid change of direction or awkward landing can affect the ligaments, menisci, cartilage or bones. These injuries can happen during football, rugby, netball, tennis, skiing, running and gym training.

MRI may be considered when a clinician suspects damage inside the joint and the result could influence rehabilitation, treatment planning or referral to a specialist. For example, an ACL injury can occur alongside a meniscal tear or bone bruising. MRI allows these structures to be examined together.

Not every sports injury happens during one obvious event. Repeated running, cycling, jumping or strength training can also irritate tendons and other tissues around the knee. Ultrasound may be suitable for some superficial tendon problems, while rehabilitation or physiotherapy may be recommended without imaging.

A knee scan cannot decide by itself when it is safe to return to sport. That decision also depends on pain, movement, strength, stability and clinical assessment.

What If Your Knee X-Ray Is Normal but Pain Continues?

An X-ray can help identify fractures, alignment problems and signs of osteoarthritis. However, it does not show ligaments, menisci and cartilage in the same detail as MRI.

If pain, swelling or instability continues despite a normal X-ray, MRI may be considered to investigate a possible soft-tissue injury, bone bruise or subtle problem inside the joint.

Persistent pain does not automatically mean that another scan is required. Further imaging is most useful when the result is likely to change how the problem is managed.

What Does Fluid on a Knee MRI Mean?

Swelling inside the joint is often described as an effusion or “fluid on the knee”. It may develop following an injury or alongside arthritis, inflammation or another condition.

MRI can show the amount and location of the fluid and may identify other findings that help explain why it is present. An effusion is a finding rather than a complete diagnosis, so it needs to be considered alongside your symptoms and clinical history.

A Baker’s cyst is a fluid-filled swelling behind the knee. MRI can identify one, but a knee ultrasound scan may also be suitable when the main concern is a cyst, superficial tendon, bursa or fluid around the joint.

What Do Knee Locking, Catching and Giving Way Mean?

People may use these words to describe different sensations.

True locking means the knee becomes physically stuck and cannot fully bend or straighten. Catching may feel like a brief obstruction during movement. Giving way can indicate instability, although pain or muscle weakness can also cause the knee to buckle.

MRI may help if a clinician suspects a meniscal tear, ligament injury, cartilage damage or loose fragment inside the joint.

A suddenly locked knee should receive prompt clinical assessment rather than waiting for a routine scan appointment.

Can Knee Arthritis Be Seen on MRI?

Knee arthritis may appear on MRI as cartilage loss, changes in the bone beneath the cartilage, bone spurs or inflammation of the joint lining.

However, MRI is not routinely needed to diagnose uncomplicated osteoarthritis. A clinical assessment may provide enough information, and an X-ray is commonly used when imaging is required.

Changes visible on a knee scan do not always match the amount of pain a person experiences. This is why imaging findings should be interpreted alongside symptoms and physical examination.

Common Knee MRI Report Terms Explained

Radiology reports use precise medical language. Some common terms include:

Chondral defect A localised area of damage affecting the articular cartilage

The wording alone cannot show how much a finding contributes to your symptoms or which treatment you need. Your GP, physiotherapist or specialist can interpret the report in the context of your history and examination.

What Does a Healthy Knee MRI Look Like?

A healthy knee MRI may show intact ligaments and menisci, preserved cartilage and no significant bone or soft-tissue abnormality.

However, there is no single image of a “perfect” knee. Minor or age-related changes can appear in people who have no pain. Comparing your images with knee MRI pictures online may therefore cause unnecessary concern.

A radiologist examines the complete series of images and considers which findings may be clinically relevant.

What Happens During a Knee MRI Scan?

How Should You Prepare for knee scan?

Before the scan, you will complete an MRI safety questionnaire. Tell the team about any medical device, previous surgery, implant or possible metal fragment in your body.

Choose loose clothing without metal zips, buttons or fasteners. You may be asked to change into a gown, and metal or electronic items must remain outside the MRI room.

What Happens Inside the MRI Room?

You will lie on the scanning table with your knee placed inside a specialist support called a coil. This helps produce detailed images of the joint.

The scanner makes tapping and buzzing sounds, so you will receive hearing protection. The radiographer can see, hear and speak to you throughout the examination.

You will need to keep your knee still while the images are taken.

How Long Does a Knee MRI Take?

Can You Have an MRI After Knee Replacement Surgery?

Many modern orthopaedic implants can be scanned safely, but the MRI team must check the specific implant before your appointment. Metal can also affect image quality around the knee replacement.

Tell the team about your surgery and provide information about the implant where possible. They will confirm whether MRI is safe and whether it is likely to answer the clinical question.

When Do Knee Symptoms Need Urgent Assessment?

Seek urgent medical assessment rather than waiting for a private knee scan if:

  • The knee is visibly deformed after an injury
  • You cannot bear weight following significant trauma
  • The knee is hot, red and very swollen, particularly if you have a fever or feel unwell
  • Pain or swelling is rapidly worsening
  • Your foot becomes cold, pale, numb or weak after an injury
  • You develop sudden swelling in your calf

Call 999 if calf swelling is accompanied by chest pain or difficulty breathing.
