Rotator Cuff Ultrasound

The rotator cuff is the most common focus of a shoulder ultrasound. It is made up of four tendons that connect the muscles of the shoulder blade to the upper arm bone, controlling movement and stabilising the joint. Rotator cuff injuries are one of the leading causes of shoulder pain in adults of all ages.

Supraspinatus

The most commonly injured rotator cuff tendon. Responsible for lifting the arm away from the body. Tears here are a frequent cause of shoulder pain and weakness.

Infraspinatus

Controls external rotation of the shoulder. Injuries are common in throwing sports and repetitive overhead activities.

Subscapularis

The largest rotator cuff tendon, responsible for internal rotation. Tears can occur following a fall or forced twisting of the arm.

Teres Minor

Works with the infraspinatus to control external rotation. Less commonly injured but assessed as part of a full rotator cuff evaluation.

The dynamic imaging advantage

Unlike MRI, an ultrasound can assess the shoulder while it is in motion. This allows real-time evaluation of the tendons and bursa during movement, which is particularly useful for diagnosing shoulder impingement and detecting tears that are only apparent in certain positions.

Shoulder conditions an ultrasound can identify

A shoulder ultrasound is the recommended first-line imaging investigation for most shoulder pain conditions. Common findings include:

Rotator Cuff Tears

Partial or full-thickness tears of the rotator cuff tendons, most commonly the supraspinatus. A frequent cause of shoulder pain and reduced strength.

Shoulder Bursitis

Inflammation of the subacromial-subdeltoid bursa, causing pain when lifting the arm. Often associated with impingement.

Shoulder Impingement

Pinching of the tendons or bursa beneath the acromion bone. Dynamic ultrasound assessment is particularly useful for diagnosing this condition.

Calcific Tendinitis

Calcium deposits within the rotator cuff tendons, which can cause sudden, intense shoulder pain. Clearly visible on ultrasound.

Biceps Tendon Problems

Tears, inflammation, or dislocation of the long head of the biceps tendon, which runs through the shoulder joint.

AC Joint Arthritis

Degeneration or inflammation of the acromioclavicular joint, a common source of pain at the top of the shoulder.

Frozen Shoulder

Also known as adhesive capsulitis. Ultrasound can identify thickening and changes around the joint capsule associated with this condition.

Tendinopathy

Degenerative changes in the tendons caused by overuse or ageing. Common in people over 40 and in those who perform repetitive overhead tasks.

Who is a Shoulder Ultrasound for?

Shoulder pain affects people of all ages and backgrounds. A private shoulder ultrasound at Vitalscan is suitable for anyone experiencing shoulder symptoms, including:

Sports and Active Patients

Sports injuries, rotator cuff strains, and tendon damage from repetitive overhead activity. Common in swimmers, tennis players, and those who lift weights.

Manual Workers

Repetitive lifting and overhead tasks can cause bursitis, impingement, and tendinopathy. A scan provides a fast diagnosis without a lengthy waiting list.

Over-50s

Age-related rotator cuff changes, frozen shoulder, and AC joint arthritis are increasingly common from middle age onwards. Early diagnosis supports faster treatment.

You do not need a GP referral to book. If you are unsure whether a shoulder ultrasound is appropriate for your symptoms, our team can advise you when you call.

Shoulder Ultrasound vs Shoulder MRI

Both ultrasound and MRI are used to investigate shoulder pain, but they serve different purposes. Understanding the difference helps you choose the right investigation for your symptoms.

Shoulder Ultrasound, £175

Fast, real-time imaging of soft tissues. Excellent for tendons, bursae, and dynamic assessment. Results within 3 to 5 working days.

For most shoulder pain presentations, ultrasound is the recommended first-line investigation. If your symptoms suggest a labral tear, significant bone changes, or your ultrasound findings are inconclusive, an MRI may be recommended as a next step.

Shoulder MRI

Comprehensive imaging of all joint structures including cartilage and bone marrow. Better for suspected labral tears, complex pathology, or inconclusive ultrasound findings. Current shoulder MRI pricing is shown on the shoulder MRI page. If you are unsure which investigation is right for you, contact our team and we can advise before you book.

Understanding your Shoulder Ultrasound results

Your written report is prepared by our clinical team and issued within 3 to 5 working days. It outlines the findings from each area assessed and is written to be shared with your GP, physiotherapist, orthopaedic surgeon, or sports medicine specialist.

Common findings include tendinopathy, bursitis, and partial rotator cuff tears, many of which respond well to physiotherapy or targeted treatment. If anything requires prompt attention, our team will contact you directly. Where an MRI or further investigation is recommended, this will be clearly stated in the report.

Paying for your Shoulder Ultrasound

Private shoulder ultrasound scans at Vitalscan are self-pay only at £175. We do not accept private medical insurance for this service, and the cost cannot be claimed through your insurer. Payment must be made in full at the time of booking.

Shoulder Ultrasound FAQs

A shoulder ultrasound shows the soft tissue structures surrounding the shoulder joint, including the four rotator cuff tendons (supraspinatus, infraspinatus, subscapularis, and teres minor), the long head of the biceps tendon, the subacromial-subdeltoid bursa, the acromioclavicular joint, and the glenohumeral joint. It can identify tears, tendinopathy, bursitis, calcific deposits, impingement, and inflammation.

Yes. Shoulder ultrasound is a primary investigation for rotator cuff tears and can identify both partial and full-thickness tears. It is particularly effective for assessing the supraspinatus tendon, which is the most commonly injured part of the rotator cuff. The scan also allows dynamic assessment during movement, which can reveal tears that are only visible in certain positions.

A shoulder ultrasound is fast, cost-effective, and allows real-time dynamic imaging during movement, something MRI cannot provide. It is excellent for assessing tendons, bursae, and soft tissues. A shoulder MRI offers a more comprehensive view of all joint structures, including the labrum, cartilage, and bone marrow, and is better suited to complex pathology or where ultrasound findings are inconclusive. For most shoulder pain presentations, ultrasound is the recommended first-line investigation.

A shoulder ultrasound can identify rotator cuff tears, tendinopathy, shoulder bursitis, shoulder impingement, calcific tendinitis, biceps tendon tears or dislocation, AC joint arthritis, frozen shoulder (adhesive capsulitis), and general soft tissue inflammation. It can also detect fluid collections and other structural changes around the joint.

No. You can book a private shoulder ultrasound at Vitalscan directly, without a GP referral. If you are unsure whether this scan is appropriate for your symptoms, our team can advise you when you call.

A private shoulder ultrasound at Vitalscan costs £175. This includes the scan, dynamic assessment during movement, clinical assessment of findings, and a written report issued within 3 to 5 working days.

No special preparation is required. Wear or bring loose clothing that allows easy access to the shoulder, a sleeveless top or vest is ideal. You can eat, drink, and take medications as normal before your appointment.

Yes. Ultrasound uses sound waves to produce images and does not use radiation. It is a safe, well-established diagnostic tool used routinely across all areas of medicine.

Most shoulder ultrasound appointments at Vitalscan take around 20 to 30 minutes, including dynamic assessment during movement.
